What to Bring?

What to bring for your initial consultation?

Here is checklist for your initial consultation:

  • Driver’s License or a valid ID
  • Active insurance card
  • Referral Letter (if required)
  • List of medications (if any)
  • Any imaging (X-rays, MRIs, CT scans etc.) including both the images (films or CD/DVD) and report

We also encourage you to come to your initial consultation with a written list of questions to ensure you don’t forget to ask them when you are seeing the doctor.


Come early for X-rays!

Please note, radiographs (X-rays) are the best initial imaging assessment for most orthopedic problems. They are often sufficient for diagnosis, and are almost always required before more advanced imaging such as MRI’s or CT scans can be obtained through your insurance provider. If you don’t have recent X-rays of the body part of interest within the last 6 months, you will be asked to go for X-rays in our office prior to being seen by Dr. Cooper. Please plan to arrive 45 minutes prior to your scheduled appointment to allow time for this.
